This is a fully remote position, open to applicants in California.
• Assess patients’ functional abilities, mobility, muscle strength, gait, and range of motion to create customized treatment plans.
• Provide physical therapy services in alignment with the physician’s care plan and in compliance with California’s Physical Therapy Practice Act.
• Guide patients and their families or caregivers on therapeutic exercises, proper equipment usage, and recovery objectives.
• Work collaboratively with physicians and the healthcare team, communicating any changes in patient status and participating in case discussions.
• Supervise Physical Therapy Assistants and, when physical therapy is the only skilled service, manage Home Health Aide care every two weeks.
• Oversee documentation, including clinical progress notes and discharge planning.
• Recognize the need for additional home health services and coordinate referrals accordingly.
• Carry out other responsibilities as necessary for patient care.
• Bachelor’s or Master’s degree in Physical Therapy from an accredited institution.
• Active Physical Therapy license in California.
• At least 2 years of experience in physical therapy; home health experience is advantageous.
• Excellent communication skills with the ability to engage effectively with patients and their families.
• Current CPR certification from the American Heart Association.
• Valid driver’s license and a dependable, insured vehicle.
