Pharmacy Technician – Care Team

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Facilitate patient care by coordinating with care team pharmacists centrally.

• Enhance medication management and streamline patient care workflows.

• Input data into the EMR and review patient records to identify and rectify potential discrepancies.

• Handle provider communications, intake patient lab results, and track patient appointments and follow-ups.

• Work collaboratively with the interdisciplinary team on medication-related matters.

• Engage with patients via phone or the EMR portal regarding their medication needs and the coordination of the pharmacist’s plan.

• Educate patients about medication interactions, dietary instructions, and potential adverse reactions.

• Assess and facilitate patient enrollment in medication assistance programs.

• Assist pharmacists with administrative responsibilities.

• Identify and escalate medication-related issues that fall outside established protocols.

• Streamline pharmacy service workflows and processes.

• Ensure compliance with care team pharmacy policies and procedures.

• Keep precise documentation within the EMR.

• Support the development of reports and tracking of service metrics.

• Contribute to ad hoc projects and other assigned responsibilities.

⛳️ Requirements

• Current Certified Pharmacy Technician (CPhT) certification.

• A valid and unrestricted pharmacy license or requisite training/certification as per state regulations.

• A minimum of 3 years of experience in a pharmacy technician position.

• Exceptional written and verbal communication abilities.

• Capability to read, write, perform mathematical calculations, multitask, and thrive in a high-pressure environment.

• Ability to work autonomously with limited supervision.

• Strong interpersonal, analytical, and problem-solving capabilities.

• Proficiency in Microsoft Office (Word, Excel) is preferred.

• Prior experience with pharmacy technician protocols is preferred.

• Familiarity with EMR or pharmacy systems is preferred.
