Pharmacist – Specialty, Call Center

This is a fully remote position, open to applicants in New Hampshire.

📋 Description

• Engage with patients and healthcare professionals to optimize therapeutic results.

• Dispense medications while overseeing patients’ drug therapies.

• Operate under collaborative practice agreements or established protocols.

• Offer consultation and education to patients, caregivers, healthcare professionals, and staff.

• Collect thorough medication histories and reconcile medication lists.

• Conduct comprehensive medication reviews and provide evidence-based recommendations.

• Identify and record suspected adverse drug reactions, medication adherence issues, and barriers to adherence.

• Communicate with providers regarding potentially unsafe or inappropriate medication use, documenting clinical activities in the electronic record.

• Act as a consultation resource for drug therapy and drug products.

• Assist with discharge planning and help find medication supplies.

• Develop, maintain, and manage medication-related guidelines in accordance with protocols or collaborative practice agreements.

• Identify and report medication errors and near misses.

• Ensure preparedness for regulatory and accreditation surveys.

• Engage in quality improvement and medication use review activities.

• Prepare and present evidence-based best-practice presentations to healthcare professionals.

• Address patient complaints and proactively devise solutions.

• Facilitate communication between departments and teams to promote optimal patient outcomes.

• Participate in initiatives and programs aimed at enhancing patient care, medication compliance, adherence, and efficiency.

• Take part in ordering and receiving controlled substances and other medications.

• Supervise and guide technicians to maintain an accurate and efficient pharmacy system.

• Serve as pharmacist-in-charge in the absence of pharmacy managers and supervisors.

• Fulfill registered pharmacist duties in New Hampshire, including administering approved vaccinations.

• Provide medical assistance as required by job responsibilities.

• Perform additional duties as assigned.

⛳️ Requirements

• Must possess a current license to practice in the state of NH.

• Must hold an unrestricted pharmacist license to practice in the state of NH.

• Must be open to pursuing license reciprocity in multiple states if necessary for the position.

• Doctor of Pharmacy (Pharm D) degree is preferred.

• Completion of a post-graduate residency and/or board certification is preferred.

• Ability to work independently with minimal supervision.

• Capable of setting priorities, making critical decisions, responding swiftly to emergencies, managing frequent interruptions, and adapting to workload changes.

• Customer-service oriented to foster excellent working relationships.

• Strong written and verbal communication skills are essential.

• Ability to perform registered pharmacist duties in New Hampshire, including administering approved vaccinations.

• Bloodborne pathogen training is mandatory.

• Ability to pursue and maintain necessary certifications for system, department, accreditation, and standards compliance.
