This is a fully remote position, open to applicants in United States.
• Deliver clinical care to a home-based patient demographic in partnership with the primary care team through thorough medication management.
• Utilize evidence-based medication utilization strategies to enhance patient and population health outcomes via medication therapies, transitions of care, and disease-management pathways.
• Detect and address potential drug therapy issues by examining patient records and available data.
• Engage in comprehensive medication evaluations for high-risk and other targeted patient groups.
• Formulate and execute monitoring and treatment plans alongside the interdisciplinary care team.
• Implement evidence-based adjustments to drug therapy under collaborative agreements for chronic disease management.
• Educate patients and healthcare providers on optimizing medication therapy.
• Conduct follow-ups with patients to monitor disease management, quality, safety, and adherence.
• Assess the appropriateness and cost-effectiveness of drug therapies.
• Maintain communication with patients, healthcare providers, staff, service teammates, and pharmacies.
• Enhance pharmacist service workflows and processes.
• Ensure compliance with medication-management guidelines, policies, and procedures.
• Keep precise documentation in the EMR.
• Support report creation and service-metric tracking.
• Report adverse drug events and engage in medication quality-improvement initiatives.
• Provide training for practitioners and staff on evidence-based medication practices and pharmacy updates.
• Assist clinical, medical, service, and operations partners with various projects as needed.
• Participate in organizational committees and develop medication-management pathways and policies.
• Execute other assigned duties.
• Doctor of Pharmacy (PharmD).
• Completion of advanced training (PGY1/PGY2) or equivalent experience (3 to 5 years) in an ambulatory care environment.
• Current and unrestricted pharmacist license(s) in the applicable state(s).
• Strong proficiency with Visio, SharePoint, MS Teams, and the Office Suite (including Excel and Access).
• Exceptional written and verbal communication skills with a proven ability to establish effective communication with staff at all levels.
• Skilled in reading, writing, mathematics; capable of multi-tasking; able to perform well in high-pressure environments.
• Ability to work autonomously with minimal supervision.
• Strong interpersonal, analytical, and problem-solving abilities.
• Preferred: Board Certification (BCPS, BCACP, BCGP, etc.).
• Preferred: Experience in caring for patients with renal disease, behavioral health needs, and diabetes/obesity.
• Preferred: Knowledge of value-based care initiatives, ACO, MSSP, Stars, and/or HEDIS measures.
• Preferred: Familiarity with EMR systems.
• Preferred: Experience in a healthcare clinic or primary care setting.
