This is a fully remote position, open to applicants in United States.
β’ Deliver clinical care to patients in a home-based setting in partnership with the primary care team via a centralized anticoagulation program.
β’ Implement evidence-based medication strategies to improve patient and population health outcomes through effective anticoagulation management.
β’ Examine patient records and information to identify and address potential medication therapy issues.
β’ Optimize, recommend, and prescribe/adjust medications in accordance with collaborative practice agreements.
β’ Educate patients and providers regarding anticoagulation therapy.
β’ Ensure patient follow-up and monitoring to enhance disease management, quality, safety, and adherence.
β’ Assess the appropriateness and cost-effectiveness of medication therapy and resolve any medication-related concerns.
β’ Communicate effectively with patients, providers, staff, service teammates, and pharmacies.
β’ Enhance the workflows and processes of the anticoagulation service.
β’ Adhere to established anticoagulation guidelines, policies, and procedures.
β’ Maintain precise documentation within the electronic medical record (EMR).
β’ Assist in the development of reports and tracking of service metrics.
β’ Report adverse drug events and engage in medication quality improvement initiatives.
β’ Provide training and education on evidence-based medication practices and pharmacy updates.
β’ Support ad hoc projects and responsibilities for service, clinical, medical, and operations teams.
β’ Doctor of Pharmacy (PharmD) degree.
β’ Current and unrestricted pharmacy license.
β’ Minimum of 2 years of experience in clinical pharmacy, with at least 1 year in a role supporting anticoagulation services.
β’ Strong proficiency in Visio, SharePoint, MS Teams, and the Microsoft Office Suite (including Excel and Access).
β’ Exceptional written and verbal communication skills, with a proven ability to establish effective communication channels with all staff levels.
β’ Skilled in reading, writing, and mathematics; capable of multi-tasking and thriving in a high-pressure environment.
β’ Ability to work independently with minimal oversight.
β’ Strong interpersonal, analytical, and problem-solving abilities.
β’ Completion of advanced training (PGY1/PGY2) is preferred.
β’ Certification for anticoagulation (CACP) is preferred.
β’ Board Certification (BCPS, BCACP, BCGP, etc.) is preferred.
β’ Familiarity with value-based care initiatives, ACO, MSSP, Stars, and/or HEDIS measures is preferred.
β’ Experience with EMR systems is preferred.
β’ Background in a healthcare clinic or primary care setting is preferred.
