Pharmacist – Anticoagulation

This is a fully remote position, open to applicants in United States.

📋 Description

• Deliver clinical care to patients in a home-based setting in partnership with the primary care team via a centralized anticoagulation program.

• Implement evidence-based medication strategies to improve patient and population health outcomes through effective anticoagulation management.

• Examine patient records and information to identify and address potential medication therapy issues.

• Optimize, recommend, and prescribe/adjust medications in accordance with collaborative practice agreements.

• Educate patients and providers regarding anticoagulation therapy.

• Ensure patient follow-up and monitoring to enhance disease management, quality, safety, and adherence.

• Assess the appropriateness and cost-effectiveness of medication therapy and resolve any medication-related concerns.

• Communicate effectively with patients, providers, staff, service teammates, and pharmacies.

• Enhance the workflows and processes of the anticoagulation service.

• Adhere to established anticoagulation guidelines, policies, and procedures.

• Maintain precise documentation within the electronic medical record (EMR).

• Assist in the development of reports and tracking of service metrics.

• Report adverse drug events and engage in medication quality improvement initiatives.

• Provide training and education on evidence-based medication practices and pharmacy updates.

• Support ad hoc projects and responsibilities for service, clinical, medical, and operations teams.

⛳️ Requirements

• Doctor of Pharmacy (PharmD) degree.

• Current and unrestricted pharmacy license.

• Minimum of 2 years of experience in clinical pharmacy, with at least 1 year in a role supporting anticoagulation services.

• Strong proficiency in Visio, SharePoint, MS Teams, and the Microsoft Office Suite (including Excel and Access).

• Exceptional written and verbal communication skills, with a proven ability to establish effective communication channels with all staff levels.

• Skilled in reading, writing, and mathematics; capable of multi-tasking and thriving in a high-pressure environment.

• Ability to work independently with minimal oversight.

• Strong interpersonal, analytical, and problem-solving abilities.

• Completion of advanced training (PGY1/PGY2) is preferred.

• Certification for anticoagulation (CACP) is preferred.

• Board Certification (BCPS, BCACP, BCGP, etc.) is preferred.

• Familiarity with value-based care initiatives, ACO, MSSP, Stars, and/or HEDIS measures is preferred.

• Experience with EMR systems is preferred.

• Background in a healthcare clinic or primary care setting is preferred.
