This is a fully remote position, open to applicants in Missouri.
• Deliver direct support services to NHS patients, including those in public schools.
• Utilize personal recovery experiences to assist and guide individuals in their treatment and recovery journeys.
• Collaborate effectively with NHS providers and staff to achieve organizational objectives, policies, and regulatory requirements.
• Conduct intake interviews with patients and evaluate social determinants of health as well as obstacles to treatment and recovery.
• Create patient-centered plans that focus on individual strengths and goals.
• Perform follow-ups with patients through phone calls, home visits, school visits, clinic visits, or other environments.
• Support patients in completing applications and registration forms.
• Identify, inform patients about, and connect them with suitable community resources.
• Work together with clinical staff and community partners.
• Relay updates on patient and family status and progress to the care team.
• Document patient care management details in the EMR or specified software within 24 hours of patient interaction, when applicable.
• Oversee a designated caseload of patients.
• Participate in staff meetings, trainings, and other requested gatherings.
• Carry out additional tasks as assigned.
• High school diploma or equivalent is required; an Associate’s or Bachelor’s Degree in a human services program is preferred.
• Certification from the Missouri Credentialing Board for Peer Support is required and must be obtained within 2 months of employment if not already acquired.
• Community Health Worker certification is to be obtained within one year upon request from a supervisor, if applicable.
• 1 to 2 years of experience in a community-based environment is preferred.
• Familiarity with some medical terminology is preferred.
• A valid driver’s license and proof of insurance for personal vehicle use for work purposes.
• Basic computer proficiency.
• Capability to work with individuals of all ages functioning at various levels.
• A suitable, quiet, and confidential remote workspace free from distractions.
• Flexibility to adjust to changing agency needs.
• Willingness to share a personal recovery narrative involving substance use and/or mental health challenges.
• Physical ability to meet requirements such as talking, seeing, hearing, reacting, standing, kneeling, walking, and sitting for extended periods.
• Proficient in reading and interpreting both oral and written instructions.
• Possess patience, kindness, compassion, along with strong written and verbal communication skills.
• Commitment to maintaining patient confidentiality.
• Ability to initiate and foster positive working relationships.
• Professional appearance and demeanor.
• Adherence to the Missouri Credentialing Board’s Certified Peer Specialist Code of Ethics and any other relevant certification ethical standards.
• Knowledge of SAMHSA’s Core Competencies for Peer Workers.
