This is a fully remote position, open to applicants in New Jersey.
• Support care coordination initiatives and address the needs of a caseload of assigned patients.
• Work alongside cross-functional teams to address challenges and meet goals.
• Serve as a communication bridge between providers and patients by transmitting messages, processing referrals and faxes, and managing prior authorizations.
• Assess and review the effectiveness of treatments and interventions, suggesting adjustments to care plans as needed.
• Partner with the integrated care team to resolve medical issues.
• Champion patients’ welfare and offer evaluations and recommendations during High-Risk Huddles.
• Reach out to patients who have been discharged from hospitals or emergency rooms to gather information and close care gaps.
• Identify, document, and address issues, objectives, and interventions within care plans.
• Relay clinical information and instructions to patients, caregivers, facilities, and other stakeholders on behalf of providers.
• Engage in collaborative care teams and High-Risk Huddles to enhance quality and patient experience outcomes.
• Recognize and ensure resolution of Social Determinants of Health barriers.
• Actively monitor patient care through outreach and check-ins according to care plans.
• Accurately document calls and patient interactions in the EHR in compliance with clinical documentation standards and relevant regulations.
• Maintain clear, compassionate, and professional communication with patients, their families, care teams, and external organizations.
• Identify, acknowledge, escalate, and resolve patient grievances, complaints, and concerns.
• Comply with HIPAA regulations and all applicable state and federal laws.
• High school diploma or GED.
• 2+ years of experience in care coordination or healthcare.
• Knowledge of Medicare, Medicare Advantage, and Medicaid insurance programs.
• Highly organized, detail-oriented, and capable of multitasking in a fast-paced, dynamic environment.
• Excellent interpersonal communication and active listening abilities.
• Adaptability to changing community needs and program conditions.
• Strong critical thinking skills and sound judgment.
• Effective time management capabilities.
• Deep commitment to patient advocacy.
• Self-motivated with strong problem-solving abilities.
• Familiarity with basic medical terminology.
• Ability to work autonomously as well as part of a team.
• Proficiency in EMR software/applications, call center technology, and the Microsoft Office Suite.
• Experience in a community or healthcare setting.
• Understanding of fundamental principles, practices, and techniques in primary healthcare.
• Active LPN/LVN license in the state where patients are located, or qualifications as a Foreign Nursing Graduate, Foreign Medical Graduate, or Community Health Worker certification (preferred).
• Familiarity with Athena (EHR) (preferred).
• 2+ years of delivering outstanding customer service (preferred).
• 2+ years in a role requiring critical thinking (preferred).
• Bilingual in Spanish and English (preferred).
