This is a fully remote position, open to applicants in Tennessee.
• Act as the primary clinical liaison for patients throughout their care journey, offering guidance and support from initial engagement to recovery and program completion.
• Address patient concerns or symptoms that may arise outside regular provider office hours.
• Provide clinical triage and guidance, which includes: self-care instructions, urgent care referrals, and escalation to on-call physicians or clinical teams.
• Offer reassurance and coaching to patients experiencing symptoms or uncertainty during overnight hours.
• Conduct proactive outreach calls to monitor patient progress and ensure adherence to care plans.
• Deliver coaching, education, and support to patients and families navigating intricate healthcare systems.
• Serve as a consistent point of contact to help patients prevent unnecessary emergency department visits or hospital readmissions.
• Perform structured post-discharge follow-up and monitoring to detect early signs of complications.
• Ensure that patients attend timely follow-up appointments with primary care providers and specialists.
• Address issues related to medication adherence.
• Escalate clinical concerns to physicians or care teams when early warning signs are recognized.
• Document all patient interactions, risk factors, and interventions within the care management platform.
• Collaborate closely with physicians, discharge planners, case managers, and other healthcare providers to ensure continuity of care.
• Facilitate access to services such as home health, rehabilitation, transportation, and durable medical equipment.
• Connect patients with community resources and support services as needed.
• An Associate’s or Bachelor’s degree in Nursing (BSN preferred).
• An active nursing license in good standing is REQUIRED for this position.
• A minimum of 3 years of clinical nursing experience is preferred.
• Experience in care management, case management, population health, or telehealth is essential.
• Prior experience working with high-risk patient populations.
• Familiarity with value-based care models (such as ACO, Medicare Advantage, bundled payment, or risk contracts) is preferred.
• Strong clinical triage and patient assessment skills are necessary.
• Exceptional communication skills and empathy towards patients.
• Ability to quickly establish trust with patients and their caregivers.
• Strong care coordination and organizational capabilities are required.
• Comfort with remote care technologies and EMR systems is a must.
• Ability to remain calm and make decisive decisions in overnight clinical situations.
