Patient Care Coordinator – Bilingual Preferred

This is a fully remote position, open to applicants in New York.

📋 Description

• Deliver exceptional customer service and service delivery to our clients and patients by managing referrals for medical goods and services.

• Contact patients regarding new or ongoing referrals to verify demographic information, provide updates about the referral, and/or confirm the delivery of goods or services.

• Liaise with vendor partners, claims adjusters, and nurse case managers to provide updates on both new and existing referrals.

• Source vendor partners for each referral in relation to the requested goods or services.

• Responsible for handling phone calls concerning existing referrals, new referrals, or referrals from other team members.

• Provide clients with quotes for approval or follow up on previously sent quotes to secure written authorization for requested referrals, ensuring that each referral is medically necessary and approved by the claims adjuster.

• Ensure that all necessary billing information is present in each referral according to the established workflow and billing processes, and transition those referrals for daily billing.

• Manage referrals in compliance with state regulations and workers' compensation guidelines.

⛳️ Requirements

• High school diploma or GED is mandatory.

• A minimum of two (2) years of experience in healthcare network and patient care scheduling, or an equivalent combination of education and experience is required.

• Strong customer service skills are essential.

• Excellent critical thinking abilities.

• Self-starter with initiative.

• Outstanding oral and written communication skills.

• Proficient in PC usage, including Microsoft Office applications.

• Possess analytical and interpretive skills.

• Strong organizational capabilities.

• Exceptional interpersonal skills.

• Ability to create and finalize comprehensive, accurate, and constructive written reports.
