Bilingual Medical Assistant – Chronic Care Management, Remote Monitoring

This is a fully remote position, open to applicants in United States.

📋 Description

• Perform monthly check-ins for CCM patients via telephone.

• Aid in the enrollment process of patients into CCM and RPM programs.

• Accurately record patient interactions in the electronic health record (EHR).

• Review and track patient health information obtained from remote monitoring devices.

• Refer clinical issues to the designated clinical team or healthcare provider.

• Educate patients on managing chronic diseases effectively.

• Uphold HIPAA regulations and ensure the confidentiality of patient information.

• Collaborate with physician offices and care teams to enhance patient care plans.

⛳️ Requirements

• Certification as a Medical Assistant (CMA) or Registered Medical Assistant (RMA) is **Required**.

• Prior experience in **Chronic Care Management (CCM)** or **Remote Patient Monitoring (RPM)** is advantageous.

• Familiarity with electronic health record (EHR) systems is necessary.

• Exceptional phone communication and skills in engaging with patients.

• Capability to work autonomously and manage follow-ups with patients.

• Bilingual proficiency (Cantonese or Mandarin) is highly preferred.

• Strong communication and interpersonal abilities.

• Excellent documentation and organizational skills.

• A compassionate, patient-centered approach is essential.

• Ability to multitask and effectively manage patient panels.
