Palliative Nurse Practitioner/Physician Assistant

This is a fully remote position, open to applicants in Kansas, +4 more states.

📋 Description

• Deliver exceptional care to palliative patients across NJ, PA, GA, VA, OK, and KS.

• Evaluate patients’ health through comprehensive physical examinations.

• Gather, update, and analyze medical histories.

• Develop optimal care plans tailored to individual patient needs.

• Coordinate with office staff and clinical operations through internal communication platforms.

• Provide patient-focused care to a designated group of patients.

• Work collaboratively with patients’ care teams to offer palliative care suggestions and holistic, interdisciplinary care strategies.

• Engage in consultations with patients, families, and caregivers during appointments.

• Facilitate discussions concerning care goals, advance care planning, and transitioning to hospice services.

• Order essential diagnostic tests as medically required.

• Prescribe medications, necessary supplies, and medical interventions.

⛳️ Requirements

• Must be Board Certified, holding a valid NP or PA certification in NJ, PA, GA, VA, OK, and KS.

• Possess strong clinical abilities to accurately and confidently treat, assess, and diagnose patients.

• Exhibit a positive attitude and outstanding interpersonal skills.

• Demonstrate the ability to solve problems effectively and work autonomously.

• Have 2-3 years of experience in palliative care, chronic care management, or primary care with geriatric populations.

• Hospice experience is highly preferred but not mandatory.

• Candidates must inform about any current or future need for employment-based immigration sponsorship prior to extending an employment offer.
