This is a fully remote position, open to applicants in United States.
• Organize, schedule, and allocate palliative patients to healthcare providers
• Perform follow-up calls with patients and their families to gather information regarding their responses to implemented treatment plans
• Act as a primary contact, advocate, and informational resource for patients, care teams, families/caregivers, insurance providers, and community resources
• Oversee referrals to appropriate agencies that align with care-plan goals and objectives
• Assist patients in navigating healthcare-system challenges and overcoming financial or social obstacles, including accessing interpreters, transportation services, and prescription assistance
• Collaborate with field staff and leadership to provide support to patients
• At least 2 years of experience in Palliative, Hospice, or Home Care is required
• A Bachelor's degree in a health-related discipline is preferred
• Experience with medical records is preferred
• HCHB experience is strongly preferred
• Demonstrated data analysis skills
• Previous experience with an electronic health record is necessary
• Successful completion of a drug screening prior to employment
• Must be legally authorized to work in the United States
