This is a fully remote position, open to applicants in United States.
• Support the daily operations of the Journey and Palliative program.
• Organize, schedule, and allocate Journey or Palliative patients to healthcare providers.
• Conduct follow-up calls with patients and families to gather feedback on responses to set treatment plans.
• Act as a point of contact, advocate, and resource for patients, care teams, families/caregivers, payers, and community resources.
• Oversee referrals to suitable agencies to assist clients in achieving their Care Plan goals and objectives.
• Aid patients with issues related to the healthcare system and address financial or social barriers, including interpreters, transportation services, and prescription assistance.
• Assist with managing medical records.
• Exhibit outstanding customer service to both internal and external stakeholders.
• Ensure communication of HIPAA compliance, corporate compliance, and client confidentiality.
• Uphold compliance with local, state, and federal regulations.
• Adhere to the Company’s Core Values and Core Competencies.
• Carry out additional duties as assigned by the supervisor.
• At least 2 (two) years of experience in Palliative, Hospice, or Home Care is preferred.
• A Bachelor’s degree in a health-related field is preferred.
• Experience with medical records is preferred.
• Strong preference for candidates with HCHB experience.
• Excellent written and verbal communication skills.
• Demonstrated ability to work effectively in a team-oriented environment.
• Capability to take on and follow through with assigned tasks and responsibilities.
• Resourceful problem-solving skills.
• Proven ability in data analysis using an electronic health record.
• Strong organizational skills with the ability to prioritize tasks to meet agency needs.
• Intermediate proficiency in Microsoft applications: Excel, Word, Outlook, PowerPoint.
• Ability to deliver exceptional customer service and showcase strong cultural competence.
• Commitment to adhering to strict confidentiality standards.
• Must possess and maintain a valid professional license, certificate, or registration, as applicable.
• Completion of a drug screen is required prior to employment.
• Must be legally authorized to work in the United States.
