This is a fully remote position, open to applicants in United States.
• Coordinate, schedule, and assign palliative patients to healthcare providers.
• Conduct follow-up calls with patients and their families to gather data on treatment-plan responses.
• Act as a contact point, advocate, and informational resource for patients, care teams, families/caregivers, payers, and community resources.
• Manage referrals to appropriate agencies that support care-plan goals and objectives.
• Assist patients with healthcare system challenges and financial or social obstacles, including interpreters, transportation, and prescription assistance.
• Collaborate with field staff and leaders to provide support to patients.
• A minimum of 2 years of experience in Palliative, Hospice, or Home Care is required.
• A Bachelor’s degree in a health-related field is preferred.
• Experience with medical records is preferred.
• HCHB experience is strongly preferred.
• Demonstrated data analysis skills.
• Previous experience using an electronic health record.
• Successful completion of a drug screening prior to employment.
• Must be legally authorized to work in the United States.
