This is a fully remote position, open to applicants in United States.
• Identify and assess potential new residential care communities, respite care services, and franchise partners.
• Facilitate partner onboarding by gathering necessary documentation, credentialing materials, and compliance agreements.
• Keep partner records, service areas, and operational updates current in internal systems and the CMS Portal.
• Monitor contract renewals, amendments, and expirations.
• Act as the main daily contact for RCCs, respite vendors, and franchise locations.
• Oversee daily referral workflows and escalate urgent or complex cases as needed.
• Conduct outreach and check-ins with partners to foster engagement and ensure service standards are met.
• Collaborate with Care Navigation and partner organizations to resolve barriers to care.
• Record partner interactions, operational issues, and follow-up actions.
• Work alongside Care Navigation, Finance, and Technical teams on referral workflows and billing or payment inquiries.
• Create, update, and maintain SOPs, workflow guides, and escalation protocols.
• Identify operational challenges and propose process enhancements.
• Assist with new operational initiatives and system upgrades.
• Monitor referral volumes, turnaround times, utilization, and partner responsiveness.
• Generate routine performance reports and basic dashboards for leadership review.
• Assess partner, operational, and billing data for accuracy, integrity, and compliance.
• 2–3+ years of experience in operations, healthcare operations, care coordination, vendor support, or network administration.
• Experience collaborating with external partners, vendors, or providers; a background in healthcare or social support settings is preferred.
• Exceptional attention to detail and the ability to manage multiple operational tasks simultaneously.
• Proficiency in Excel or Google Sheets, basic dashboards, and system portals.
• Excellent written and verbal communication skills.
• Capability to work cross-functionally and manage daily tasks in a remote setting.
• Knowledge of respite care, residential care or skilled nursing care, home health, or caregiving services is preferred.
• Familiarity with contract coordination, administrative compliance, or credentialing processes is preferred.
• Must be eligible to work in the United States; Synapticure cannot provide work visa sponsorship.
• Work remotely from anywhere in the 50 United States.
