Oncology Nurse Navigator

This is a fully remote position, open to applicants in Rhode Island.

📋 Description

• Facilitate triage, case management, and care navigation for oncology patients with possible comorbidities.

• Provide direct patient care via phone, email, video, and virtual platforms.

• Offer comprehensive assistance throughout the oncology care continuum, from screening to survivorship or end-of-life care.

• Handle urgent clinical escalations and offer consultative clinical support.

• Triage symptom survey responses and incoming reported symptoms, assisting patients directly or escalating to Nurse Practitioners as necessary.

• Identify and address patient needs, including care coordination, nutritional support, discharge planning, and referrals to providers.

• Educate patients on their diagnosis, the range of care options available at Daymark Health, and the next steps in their care.

• Foster trusting relationships with patients and their support networks through empathy and active listening.

• Collaborate with Nurse Practitioners, Medical Directors, Social Workers, Clinical Operations, and non-clinical Health Partners.

• Address social determinants of health, including food security and access to transportation.

• Adhere to Daymark policies, procedures, and documentation standards while optimizing the use of systems, tools, and technology.

⛳️ Requirements

• Bachelor of Science Degree in Nursing is required.

• Compact unrestricted registered nurse (RN) license, or the ability to obtain one.

• Licensure in Rhode Island is required, or willingness to obtain licensure in Rhode Island.

• Minimum of 3 years of nursing experience, including at least 2 years in oncology nursing.

• A strong passion for assisting patients in understanding the advantages of palliative care.

• Oncology-related certifications such as OCN, AOCN, AOCNS, or CCM are strongly preferred, or must be obtained within 2 years of hire.

• Proficiency with Slack, Zoom, Google Suite, and EHR systems is essential.

• Excellent relational, communication, organizational, and adaptability skills are required.

• Capability to multitask in fast-paced environments while maintaining meticulous organization in communications and documentation.

• Basic Life Support (BLS) certification is required for patient-facing roles by the start date.

• Compliance with Daymark's Employee Vaccination and Testing policy is mandatory by the start date.
