This is a fully remote position, open to applicants in California.
β’ Deliver comprehensive medication management services, which include reconciliation, dose optimization, adherence support, and drug-level evaluations based on evidence-based practices.
β’ Collaborate with physicians, specialists, and interdisciplinary teams to create personalized care plans under a Collaborative Practice Agreement (CPA) and facilitate smooth medication coordination.
β’ Assess the effectiveness and safety of therapies, identify opportunities for optimization, and intervene as necessary.
β’ Participate in quality improvement and Risk Evaluation and Mitigation Strategy (REMS) initiatives.
β’ Offer operational recommendations by assisting team members with complex prior authorizations, enhancing workflows, educating patients, and ensuring ongoing clinical development.
β’ Possess a Doctor of Pharmacy (PharmD) degree.
β’ Must be a licensed pharmacist in California.
β’ Completion of a PGY2 Oncology Residency or PGY1 Ambulatory Care Residency, along with 3 years of experience as a clinical oncology pharmacist, or a minimum of 3 years of clinical pharmacy experience in oncology, including at least 1 year practicing under a CPA.
β’ Excellent communication, organizational, and technological skills.
