This is a fully remote position, open to applicants in United States.
• Communicate with patients via phone to provide Arine's clinical services, which include conducting Medication Therapy Management (MTM) services such as Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews (TMRs), medication adherence counseling, medication reconciliation, and clinical program education on behalf of their health plan.
• Review completed medication reviews and counseling performed by other Virtual Pharmacist users as assigned.
• Analyze, assess, and document patient data in Arine's software platform and other systems; prepare written reports and provide oral reports on patient care and team progress.
• Follow up with clinics, providers, pharmacies, and patients to promptly resolve patient issues and concerns, including transitions of care.
• Create intervention strategies to enhance medication utilization, fulfill quality measures, and achieve other established objectives.
• Handle a steady volume of interactions with patients, providers, and pharmacies while sustaining productivity and service quality.
• Meet individual and team performance metrics concerning outreach, engagement, documentation quality, and clinical outcomes.
• Adapt and rotate among clinical projects and assignments based on changing business and client requirements.
• Identify opportunities to enhance workflows and Arine's software platform by participating in meetings, collaborating across functions, submitting tickets, and utilizing automation and technology to boost efficiency.
• Provide daily sign-out and maintain clear communication for handoffs to ensure continuity of care.
• Coach and support Clinical Care team members and guide new Virtual Pharmacist users through workflows and the platform.
• Doctor of Pharmacy (PharmD) from an accredited college or university.
• Licensed Pharmacist in good standing (multiple state licenses are advantageous).
• Familiarity with CMS quality measures, Star Ratings, HEDIS metrics, and Medicare Part C and Part D medication measures.
• Understanding of chronic disease states across various therapeutic areas (e.g., cardiology, pulmonology, endocrinology, behavioral health), including management of patients with polypharmacy.
• Knowledge of prescription and non-prescription drugs and drug products along with their indications, efficacy, dosage, mechanisms of action, major contraindications, and potential side effects.
• Capability to review and evaluate the appropriateness of prescriptions, taking into account factors such as patient age, sex, concurrent disease states, concurrent drug therapy, foods, and diagnostic procedures affecting drug activity and disposition.
• Proficient in engaging with patients, promoting adherence, and utilizing motivational interviewing and counseling techniques.
• Exceptional written and oral communication skills, along with the emotional intelligence to build rapport, navigate sensitive interactions, and maintain a patient-focused approach.
• Experience working with patients from diverse cultural, religious, and socio-economic backgrounds.
• Proven reliability, accountability, and follow-through.
• Strong attention to detail with a commitment to documentation accuracy.
• Comfortable working in a metrics-driven environment where outcomes and quality are monitored.
• Ability to work independently, exercising sound judgment, organizing and prioritizing competing responsibilities, and meeting deadlines with minimal supervision.
• Capacity to work effectively within a team, fostering productive working relationships and building consensus.
• Willingness to accept coaching, feedback, and engage in continuous learning.
• Ability to adapt and excel in a fast-paced environment, managing multiple Medicare measures and clinical priorities while navigating shifting workflows and technologies.
• Awareness of local, state, and federal regulatory requirements relevant to functional responsibilities.
• Strong computer skills, including proficiency in Google Workspace and Mac (Apple) environment, with ease in using collaboration and contact-center tools such as Slack and Five9, navigating multiple browser-based systems concurrently, utilizing keyboard shortcuts, maintaining a high typing speed (WPM), and performing basic technical troubleshooting to remain productive in a phone-based setting.
• Willingness and ability to obtain a registered pharmacist license in other states as necessary.
• Availability to work 40 hours per week (within company operating hours of Monday-Friday, 6AM-6PM Pacific Time, with up to two rotating Saturday shifts per month).
