Clinical Pharmacy Specialist

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver thorough medication management services, which include reconciliation, dose optimization, adherence support, and drug-level evaluations, all in accordance with evidence-based practices.

• Collaborate with physicians, specialists, and multidisciplinary teams to develop personalized care plans under a Collaborative Practice Agreement (CPA) and facilitate effective medication coordination.

• Assess the effectiveness and safety of therapies, identify opportunities for optimization, and intervene as necessary.

• Contribute to quality improvement initiatives and Risk Evaluation and Mitigation Strategies (REMS).

• Offer operational recommendations by assisting colleagues with complex prior authorizations, enhancing workflows, educating patients, and fostering ongoing clinical development.

⛳️ Requirements

• Possess a Doctor of Pharmacy (PharmD) degree.

• Must hold a valid pharmacist license in California.

• The candidate should fulfill one of the following criteria:

• Completion of a PGY2 Ambulatory Care Residency.

• Completion of a PGY1 Residency along with a minimum of 1 year of ambulatory practice under a Collaborative Practice Agreement (CPA).

• At least 3 years of ambulatory practice experience with a focus on disease state management.

• Excellent communication, organizational, and technical skills.

• Demonstrate empathy, integrity, and a collaborative team-oriented approach.
