This is a fully remote position, open to applicants in Washington.
• Evaluate and screen patients' daily living and work-related abilities and create therapeutic retraining programs with measurable goals.
• Conduct and interpret diagnostic and prognostic functional assessments in patients' homes or various environments.
• Collaborate with physicians and healthcare team members to develop and modify Plans of Care Treatment.
• Deliver therapeutic treatment and guidance to enhance or restore strength, coordination, range of motion, and functionality, or instruct on compensatory strategies.
• Review and enhance retraining programs in alignment with each patient's behavioral objectives and the overall rehabilitation strategy.
• Consult with vocational team members to formulate and execute vocationally focused plans.
• Communicate patient status and progress to physicians, rehabilitation staff, Clinical Manager, patients, and family members.
• Engage in care coordination and discharge planning, supporting other healthcare personnel in fulfilling patient needs.
• Create community reintegration activities and align plans with the interdisciplinary team.
• Recommend or design adaptive equipment to enhance home or work settings and aid participation in rehabilitation.
• Educate staff and family members on retraining programs, including skills for dressing, feeding, grooming, and hygiene.
• Oversee, instruct, and assess the performance of Occupational Therapy Assistants and Home Health Aides.
• Record patient care observations, interventions, and evaluations on the day services are provided.
• Accurately document and implement interim physician orders.
• Submit evaluations, treatment plans, and discharge summaries to supervisors and care management personnel.
• Valid and unrestricted occupational therapist licensure.
• Current CPR certification.
• Valid driver's license.
• Auto insurance that meets the minimum requirements of the residing state or the specified Humana limits, whichever is greater.
• Reliable means of transportation.
• Preferred minimum of six months of occupational therapy experience.
• Previous home health experience is preferred.
• Capability to undergo TB screening if selected.
• Availability for 40 hours per week.
