Nurse Practitioner / Physician Assistant – Hawaii Licensed

This is a fully remote position, open to applicants in Hawaii.

📋 Description

• Perform thorough AWVs for Medicare beneficiaries using secure telehealth platforms.

• Lead organized ACP discussions, recording patients’ objectives and preferences for care.

• Assess medical histories, medications, functional and cognitive abilities, and preventive care requirements.

• Detect and address care gaps by advising patients on screenings, vaccinations, and chronic disease management.

• Work in collaboration with interdisciplinary care teams and primary care providers to guarantee coordinated, patient-focused care.

• Ensure timely and accurate documentation within the EHR system.

• Comply with CMS regulations and clinical quality benchmarks.

⛳️ Requirements

• Possession of an active, unrestricted NP or PA-C license in Hawaii.

• A minimum of 3 years of clinical experience, including AWVs and ACP.

• In-depth knowledge of Medicare regulations and preventive care protocols.

• Skilled in using virtual care technology and EHR systems.

• Exceptional communication and interpersonal abilities.

• Bilingual in Tagalog or Ilocano is advantageous (not mandatory).

• Capability to work independently and manage a flexible virtual schedule.
