Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Maryland.

📋 Description

• Conduct in-home health assessments as a Nurse Practitioner or Physician Assistant

• Interact with health plan members in their residences and establish trusted relationships

• Perform visits lasting approximately 45 minutes

• Evaluate medication usage and medical history

• Carry out physical examinations

• Administer additional diagnostic tests upon the health plan's orders, including diabetic eye exams and spirometry

• Utilize an iPad and follow clinical workflows for each evaluation

• Facilitate virtual visits as necessary

• Offer Diagnostic & Preventive Services, such as spirometry, diabetic eye exams, and bone density screenings

• Travel locally and within licensed states as required

• Assist in bridging gaps in care and connecting members to suitable services

⛳️ Requirements

• Possession of an active, unrestricted license(s) in relevant coverage areas

• Board certification is mandatory

• Multi-state licenses are advantageous

• Availability to work 40 hours per week

• Willingness to travel locally and within licensed states as necessary

• Capability to perform visits in private homes that may have pets, secondhand smoke, and airborne allergens or irritants
