Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Washington.

📋 Description

• Conduct in-home health evaluations lasting around 45 minutes.

• Build trusted relationships while interacting with health plan members in their residences.

• Review patients' medications and medical histories.

• Perform physical examinations.

• Execute additional diagnostic tests as prescribed by the health plan, such as diabetic eye assessments and spirometry.

• Utilize an iPad and clinical workflow for every evaluation.

• Travel locally and within licensed states as required.

• Conduct virtual visits when necessary.

• Provide Signify Health services, encompassing Diagnostic & Preventive Services like spirometry, diabetic eye exams, and bone density screenings.

• Assist in identifying connections to suitable care and enhancing health outcomes.

⛳️ Requirements

• Possess an active, unrestricted license(s) in the coverage areas of Seattle/Snohomish.

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Ability to safely and comfortably perform duties in homes that may have household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Qualification as a Nurse Practitioner or Physician Assistant.
