Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Washington.

📋 Description

• Conduct approximately 45-minute health evaluations in clients' homes.

• Establish trusted relationships with health plan members during in-home visits.

• Review patients' medications and medical history thoroughly.

• Execute physical assessments effectively.

• Administer additional diagnostic tests as required, such as diabetic eye examinations and spirometry.

• Utilize an iPad and clinical workflow for each evaluation conducted.

• Facilitate virtual consultations as necessary.

• Offer Signify Health services, encompassing Diagnostic & Preventive Services products.

• Assist in identifying connections to appropriate care and bridging gaps in healthcare.

• Travel locally and within licensed states as required.

⛳️ Requirements

• Possession of active, unrestricted license(s) in applicable coverage area(s).

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Capability to cover Bremerton MSA and Kitsap County.

• Willingness to travel locally and within licensed states as necessary.

• Ability to work safely and comfortably in homes that may have household pets, secondhand smoke, airborne allergens, or irritants.

• Qualification as a Nurse Practitioner or Physician Assistant.
