Nurse Practitioner or Physician Assistant – In-Home Health

This is a fully remote position, open to applicants in Minnesota.

📋 Description

• Conduct approximately 45-minute In-Home Health Evaluations at members’ residences.

• Interact individually with members and address their health inquiries.

• Assess medications and review medical histories.

• Execute physical examinations.

• Perform additional diagnostic tests as directed by the health plan, including diabetic eye exams and spirometry.

• Establish trustworthy relationships to enhance health outcomes.

• Visit health plan members across the nation and connect them with relevant care.

• Travel locally and within licensed states as necessary.

• Conduct virtual visits when required.

• Deliver Signify Health Diagnostic & Preventive Services, such as spirometry, diabetic eye exams, and bone density screenings.

• Utilize an iPad and clinical workflow for each evaluation.

• Avoid prescribing medication, ordering lab tests, or modifying existing treatment plans.

⛳️ Requirements

• Nurse Practitioner or Physician Assistant qualification.

• Active and unrestricted license(s) in relevant coverage areas.

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Willingness to travel locally and within licensed states as required.

• Capability to conduct visits in private homes that may have household pets, secondhand smoke, and airborne allergens or irritants.

• Proficiency in using an iPad and clinical workflow for evaluations.

• Ability to perform tasks safely and comfortably in various home settings.
