
Nurse Case Manager – NICU, Bilingual in Haitian Creole

This is a fully remote position, open to applicants in United States.
• Enable families of members to foster optimal wellness while advocating for essential medical services throughout the healthcare continuum.
• Operate in a telephonic managed care environment utilizing clinical expertise and critical thinking to develop care plans for high-risk members.
• Evaluate members' physical, functional, psychological, environmental, educational, and financial requirements.
• Conduct needs assessments and create personalized care plans.
• Determine risk stratification and establish ongoing call frequency.
• Oversee care plans, service delivery, benefit adequacy, and progression towards objectives.
• Collaborate with providers on care coordination tasks, such as scheduling appointments, arranging transportation, and managing durable medical equipment (DME).
• Inform members about their conditions, treatment strategies, benefits, services, and how to access care.
• Offer referrals to community resources and facilitate access and communication among various services.
• Engage in interdisciplinary and client rounds to identify, clarify, and mitigate risks, quality issues, or deviations in care plans.
• Preferred EST time zone.
• Must be bilingual in Haitian Creole.
• Registered Nurse (RN) with a current, unrestricted compact license is required.
• A minimum of three (3) years of clinical experience in a Level III or higher NICU is required.
• CCM certification is required within the timeframe outlined by company policy.
• Experience with data entry into a database or electronic health record (EHR) is required.
• Must possess computer literacy.
• Strong communication skills and organizational abilities are required.
• Must be self-driven and eager to learn various tasks.
• Should be well-organized and capable of prioritizing responsibilities.
• Must exhibit accuracy in spelling and documentation.
• Proven ability to work both independently and collaboratively within a team.
• Excellent interpersonal skills and telephone etiquette are essential.
• A commitment to excellence in customer service is required.
