Nurse Case Manager – NICU

This is a fully remote position, open to applicants in New York.

📋 Description

• Enable families of members to promote optimal wellness while advocating for essential medical services throughout the health care spectrum.

• Operate in a telephonic managed care environment to create tailored care plans for high-risk members.

• Evaluate members’ physical, functional, psychological, environmental, educational, and financial requirements.

• Conduct needs assessments and formulate personalized care plans.

• Assign risk levels and determine the frequency of follow-up calls.

• Oversee care plans, service delivery, benefits sufficiency, progress towards objectives, and variations in health status.

• Collaborate with providers to organize appointments, transportation, durable medical equipment (DME), and additional services.

• Educate members about their conditions, treatment strategies, benefits, services, and how to access care.

• Offer community resource referrals and facilitate access along with communication across various services.

• Engage in interdisciplinary and client rounds to identify, clarify, or mitigate risks, quality concerns, or discrepancies in care plans.


⛳️ Requirements

• Preferred EST or CST time zone.

• A valid, unrestricted Registered Nurse (RN) license in New York State is mandatory.

• A minimum of three (3) years of clinical experience in a Level III or higher NICU is required.

• CCM certification must be obtained within the timeframe established by company policy.

• Experience with data entry in a database or electronic health record (EHR) is essential.

• Proficiency in computer usage is necessary.

• Outstanding communication skills and organizational capabilities are required.

• Must be self-driven and eager to learn various tasks.

• Must possess strong organizational skills and the ability to prioritize effectively.

• Must exhibit accuracy in spelling and documentation.

• Ability to work independently and collaboratively within a team setting is crucial.

• Exceptional interpersonal skills and telephone manners are required.

• A strong commitment to providing excellent customer service is essential.

• A college degree is preferred.
