Nurse Case Manager

This is a fully remote position, open to applicants in Canada.

📋 Description

• Strategize and oversee intricate care coordination cases and related activities

• Ensure prompt patient access, treatment advancement, and ongoing care continuity

• Perform assessments and assist with care planning

• Manage transitions of care by addressing clinical, social, and accessibility challenges

• Collaborate with providers, pharmacies, payers, and internal teams to eliminate access obstacles

• Align subsequent actions and sustain case continuity

• Evaluate case trends, recurring problems, and workflow patterns to pinpoint root causes

• Propose enhancements to processes

• Create and improve job aids, care coordination templates, and standards for documentation

• Mentor less experienced team members in complex case management, escalation procedures, and documentation standards

⛳️ Requirements

• A bachelor's degree in nursing, social work, health sciences, public health, healthcare administration, or a related discipline, or equivalent experience is necessary

• A minimum of 4 years of experience in care management, case management, clinical support, patient services, or a similar field is required

• Certification in case management, care coordination, clinical specialty, or equivalent is preferred
