Nurse Care Manager

This is a fully remote position, open to applicants in Alaska, +9 more states.

📋 Description

• Provide tailored care management and coordination services.

• Perform patient evaluations and create personalized care plans.

• Assist in managing chronic diseases (such as diabetes, hypertension, CHF, and COPD).

• Educate patients on disease conditions, medications, and self-management techniques.

• Employ motivational interviewing methods to boost adherence and involvement.

• Work collaboratively with healthcare providers and care teams to synchronize treatment plans.

• Recognize care gaps, risks, and obstacles to achieving optimal outcomes.

• Ensure accurate, compliant, and timely record-keeping.

• Facilitate care transitions and coordination of follow-up services.

⛳️ Requirements

• Current Registered Nurse (RN) license in one of the following states: NY, NV, MI, MN, IL, ID, HI, DC, CA, AK.

• At least 2 years of experience in care management, case management, or population health.

• Capability to effectively manage a patient panel specific to the state.
