This is a fully remote position, open to applicants in United States.
• Evaluates and examines the medical, behavioral, and/or vocational condition of the injured worker, using Carisk’s patient-centered, strengths-based biopsychosocial methodology.
• Aids in the formulation of a Care Coordination plan aimed at helping the patient achieve optimal functional improvement.
• Interacts with the patient, current and prospective providers, as well as vocational and social support systems, and other external stakeholders.
• Collaborates with Carisk’s Clinical, Quality, Provider Relations, and other internal teams.
• Oversees a patient’s advancement toward the desired outcomes through a customer-facing monthly progress report.
• Organizes referrals, consultations, and therapeutic services, and collaborates with other specialists regarding the care and treatment plan.
• Diligently documents all cases, activities, and hours in accordance with customer-specific guidelines and Carisk’s documentation standards.
• Participates in all meetings and conference calls as requested by Carisk's Clinical and Administrative teams.
• Maintains any necessary credentials and complies with all ethical codes required by these credentials.
• Performs various professional responsibilities as assigned by management.
• Current, active, and unrestricted Nursing License (RN or LPN) is required.
• Experience in Workers’ Compensation and Case Management is preferred.
• A minimum of 1-2 years of relevant experience is needed.
• Certified Case Manager (CCM) designation is preferred.
• Bilingual proficiency in English and Spanish is required.
• Must be available to work from 11:30am to 8:00pm EST (all time zones accepted).
