This is a fully remote position, open to applicants in Kentucky, +4 more states.
• Perform outreach via telephone, predominantly using an auto-dialer, to members who have multiple chronic conditions along with financial or functional challenges.
• Evaluate member needs through data analysis, structured assessments, and direct discussions.
• Create and oversee personalized care plans.
• Coordinate services and link members to clinical, community, and multidisciplinary resources.
• Ensure compliance with all relevant requirements.
• Identify and address obstacles that may hinder effective care and member autonomy.
• Track progress towards achieving health goals and modify care strategies as necessary.
• Operate multiple computer systems concurrently while engaging with members in a dynamic, structured environment.
• An associate degree in any field.
• At least 2 years of experience in direct adult patient care, such as in long-term care, home and community-based services, hospital/facility settings, case management, or managed care organizations.
• Experience collaborating with members of the healthcare team.
• Proficient in computer usage.
• Capable of quickly learning and navigating electronic software applications, including electronic medical records.
• Excellent communication and interpersonal skills.
• A minimum download speed of 25 Mbps and upload speed of 10 Mbps for self-provided internet service.
• Ability to work from a dedicated area without ongoing interruptions to safeguard member PHI/HIPAA information.
