This is a fully remote position, open to applicants in Iowa.
• Design, evaluate, and implement intricate care management initiatives for members primarily with physical health needs.
• Deliver exceptional, cost-effective healthcare outcomes through customized care plans and educational resources.
• Assess member requirements, obstacles to access, and social determinants affecting health.
• Create continuous care plans while collaborating with healthcare providers, specialists, and community resources.
• Recognize issues and obstacles to care, offering suitable interventions.
• Facilitate coordination among members, families/caregivers, and care provider teams.
• Track member status, changes in condition, and progress towards care plan objectives.
• Adjust or enhance care plans as necessary to align with member goals and unmet needs.
• Offer resource assistance for services related to employment, housing, participant direction, independent living, justice, and foster care.
• Manage care coordination to ensure timely access to required care and services.
• Conduct outreach via phone, digital platforms, home visits, and/or other locations.
• Gather, record, and maintain member information and care management activities to ensure regulatory compliance.
• Educate members and their families/caregivers on disease processes, care gaps, provider instructions, care options, referrals, and healthcare benefits.
• Provide leadership insights on opportunities for enhancing care and quality delivery.
• Execute other assigned responsibilities while adhering to established policies and standards.
• Degree from an accredited School of Nursing or a Bachelor’s degree in Nursing.
• 2–4 years of relevant experience.
• RN - Registered Nurse - State Licensure and/or Compact State Licensure is mandatory.
• Preference for candidates living in Iowa.
• Preference for an active Iowa RN license.
• Preference for experience in case management and prenatal/post-partum maternal health.
• Experience with OB or NICU is advantageous.
• Availability to work Monday through Friday, 8 AM to 5 PM CT.
