Manager, Care Coaching

This is a fully remote position, open to applicants in Florida.

📋 Description

• Act as the main liaison for Care Coaches

• Ensure team adherence to compliance standards

• Collaborate with various departments

• Provide support to a team of 15–17 Care Coaches

• Oversee coaching and accountability frameworks

• Assist in generating metric-driven reports

• Recognize obstacles and resources to navigate them effectively

• Maintain appropriate caseload ratios while minimizing travel and overtime

• Supervise patient care through assessments, data analysis, member conversations, and active care planning

• Guarantee that patients achieve their desired outcomes

• Make decisions pertaining to resources, strategies, and tactical operations for departmental projects and initiatives

• Facilitate cross-departmental briefings and area meetings

• Keep regular communication with other department managers

• Report to the Regional Care Coach AD

⛳️ Requirements

• Bachelor's Degree in a health-related discipline

• A minimum of 2 years of managed care experience in Medicare and/or Medicaid

• At least 1 year of management or leadership experience

• A minimum of 1 year of experience with complex membership

• Skilled in analyzing and interpreting trends

• Progressive experience in the health solutions sector, particularly in leading and managing teams

• Valid driver's license from the state

• Proof of personal vehicle liability insurance with minimum coverage of 100,000/300,000/100,000

• Must live within 3 hours of Broward County

• Minimum home internet download speed of 25 Mbps and upload speed of 10 Mbps

• Dedicated workspace free from ongoing interruptions to safeguard member PHI/HIPAA information

• RN or LPN with current state licensure

• Master's degree in human services, social sciences, or a health-related field

• Relevant state licensure in the area of study

• Proficient in analyzing and interpreting financial trends

• Prior experience in utilization management, discharge planning, and/or home health or rehabilitation

• Experience in health promotion, coaching, and wellness

• Familiarity with health plan operations

• Previous experience with Medicare/Medicaid

• Experience in maintaining NCQA standards

• Florida Level 2 background screening through the Care Provider Background Screening Clearinghouse
