This is a fully remote position, open to applicants in Arizona, +9 more states.
• Evaluate the needs of members concerning physical health, mental health, substance use disorder (SUD), oral health, palliative care, memory care, trauma-informed care, social supports, housing, and referrals to community-based services.
• Supervise the creation of client care plans and the establishment of goals.
• Provide services in locations convenient for members, including office settings, telehealth, or field-based services.
• Link clients to necessary social services and supports.
• Advocate for clients with healthcare providers (e.g., primary care physicians, etc.).
• Apply evidence-based methods, such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles.
• Perform outreach and engagement initiatives to promote connection to the ECM program and document activities in the Client Relationship Management (CRM) system.
• Assess client progress and revise SMART goals as needed.
• Facilitate mental health promotion.
• Organize transportation (e.g., ACCESS).
• Ensure all documentation, including outcome measures, is completed within the timelines specified in individual care plans.
• Keep patient health records current in the Electronic Medical Record (EMR) system and other business systems.
• Prepare monthly reports to guarantee program compliance.
• Participate in assigned training sessions.
• Must possess an active, unrestricted Licensed Vocational Nurse (LVN) license in California.
• A minimum of 2 years of experience as a care manager, care navigator, or community health worker serving vulnerable populations.
• Familiarity with government and community resources related to social determinants of health.
• Outstanding oral and written communication skills.
• Strong interpersonal skills are essential.
• Proficiency in general computer skills and a working knowledge of Google Workspace, MS Office, and the internet.
• Bilingual in English and Spanish is highly preferred.
