This is a fully remote position, open to applicants in Florida.
• Develop, evaluate, and orchestrate comprehensive care management activities for members in long-term care.
• Create or assist in the formulation of personalized service care plans, educating members and their caregivers about available services and benefits.
• Assess member needs alongside available resources, recommending or facilitating suitable care plans.
• Identify necessary providers, specialists, and community resources for long-term care.
• Coordinate communication between members, caregivers, and care provider teams to ensure services are readily accessible.
• Provide resource support related to employment, housing, participant direction, independent living, justice, foster care, and similar needs.
• Monitor care plans, member status, and outcomes, suggesting updates based on recognized needs.
• Engage with healthcare providers and partners involved in long-term care.
• Collect, document, and maintain member information and care management activities to ensure regulatory compliance.
• Conduct home and/or other site visits to evaluate member needs and collaborate with providers and partners.
• Facilitate educational sessions on procedures, healthcare provider instructions, service options, referrals, and healthcare benefits.
• Provide leadership insights on opportunities to enhance quality of care and cost-effective service delivery.
• Carry out additional duties as assigned while adhering to policies and standards.
• Must reside in Pinellas County, Florida.
• Bachelor's degree with 2–4 years of relevant experience; or a bachelor's degree in health, human services, social work, or education services with a minimum of one year of qualifying experience.
• Alternatively, a high school diploma or equivalent with three years of qualifying experience in case management for aged, behavioral health, physical or developmental disabilities, or HIV/AIDS populations.
• Experience in interviewing and assessing member needs.
• Proficiency in caseload management and casework practices.
• Knowledge of eligibility determination for DHSS programs.
• Familiarity with federal and state laws applicable to DHSS programs.
• Strong problem-solving abilities and skill in locating community resources.
• Capacity to collaborate effectively with caregivers, state agency representatives, and providers.
• Strong interpersonal skills.
• Ability to practice cultural competency with an awareness and respect for diversity.
• Understanding of the needs and service delivery system for populations within the assigned caseload.
• Technological proficiency in field-based environments.
• Exceptional communication and customer service skills.
• Strong assessment skills.
• Prior case management experience.
• Equivalent experience may be considered if it demonstrates the required knowledge, duties, scope, and skills.
