This is a fully remote position, open to applicants in North Carolina.
• Manage a team delivering integrated, holistic care within a managed care framework.
• Oversee staff responsibilities on a day-to-day basis, including performance management, staff development, and attendance tracking.
• Ensure that staff adhere to established workflows for conducting thorough assessments and developing care plans.
• Create and sustain a team-oriented, person-centered methodology for care coordination that addresses physical, behavioral, and social health requirements.
• Track team performance metrics, distribution of caseloads, and quality indicators.
• Guarantee the delivery of quality services while ensuring compliance with regulatory and accreditation standards.
• Coordinate care for members with complex health needs.
• Offer clinical supervision and guidance, leveraging professional licensure to aid in care planning and clinical decision-making.
• Address and resolve intricate member cases.
• Provide coaching, education, and direction to staff for ongoing improvement and professional development.
• Be available to work remotely from 8:30 am to 5:00 pm and travel within the community for meetings as required.
• Associate's degree (RN only) with three (3) years of experience in care management, case management, or care coordination for the MH/SU/IDD population; OR a Bachelor's degree (RN only) with three (3) years of relevant experience; OR a Master's degree with full licensure as LCSW, LCMHC, LPA, LMFT, or RN and three (3) years of experience in similar roles.
• Must hold a valid license in the State of North Carolina as one of the following: LCSW, LCMHC, LPA, LMFT, LCAS, or RN.
• If applying with an Associate’s or Bachelor’s degree, you must be fully licensed in North Carolina as an RN.
• Possession of a valid driver's license is required.
• Candidates must reside within Trillium’s catchment area in North Carolina, which includes counties such as Anson, Bertie, Beaufort, Bladen, and many others.
• Ability to travel within the catchment area as necessary.
• All candidates must pass a drug test as a condition of employment.
• Preferred: Degree in a Human Services-related field.
• Preferred: At least one (1) year of supervisory experience overseeing two (2) or more staff members.
• Preferred: Experience with a Managed Care Organization (MCO), Health Plan, or in managed-care case management.
