Licensed Practical Nurse, LPN, LVN, Day Shift

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Collaborate effectively with providers to implement value-based care management initiatives.

• Oversee patient census with a focus on resolution to address gaps in both clinical and non-clinical patient care.

• Identify and connect patients with community resources.

• Educate patients on health literacy and the management of chronic conditions.

• Conduct medication management, recognizing potential concerns related to medications.

• Support timely service delivery for Home Health, DME, and Home Infusion.

• Address patient inquiries and oversee referrals and appointment scheduling.

⛳️ Requirements

• Minimum of 3 years of experience in direct patient-facing roles.

• Valid Multi-State/Compact License (LPN) (NCL) (LVN).

• Proficient in Microsoft Office Suite.

• Strong written and verbal communication abilities.

• Experience with Epic (preferred).

• Bilingual skills (preferred).

• Additional single state licensures (LPN) (preferred).
