Licensed Practical Nurse, LPN, LVN

This is a fully remote position, open to applicants in Texas.

📋 Description

• Oversee patient census with a focus on resolving issues to bridge gaps in both clinical and non-clinical patient care.

• Identify and organize community resources that can enhance patient care.

• Deliver education to patients and improve health literacy regarding the management of chronic illnesses.

• Conduct medication management, which includes spotting potential medication issues, reconciliation, adherence, and coordinating refills.

• Support the timely provision of services for your patients, including Home Health, DME, Home Infusion, and other essential needs.

• Address patients' inquiries and foster open communication to better understand their needs.

• Assist with managing referrals and scheduling appointments.

⛳️ Requirements

• Valid Multi-State/Compact License (LPN) (NLC) (LVN)

• Proficient in Microsoft Office Suite

• Strong written and verbal communication abilities

• Minimum of 3 years of experience in direct patient-facing roles
