This is a fully remote position, open to applicants in Texas.
β’ Oversee patient census with a focus on resolving issues to bridge gaps in both clinical and non-clinical patient care.
β’ Identify and organize community resources that can enhance patient care.
β’ Deliver education to patients and improve health literacy regarding the management of chronic illnesses.
β’ Conduct medication management, which includes spotting potential medication issues, reconciliation, adherence, and coordinating refills.
β’ Support the timely provision of services for your patients, including Home Health, DME, Home Infusion, and other essential needs.
β’ Address patients' inquiries and foster open communication to better understand their needs.
β’ Assist with managing referrals and scheduling appointments.
β’ Valid Multi-State/Compact License (LPN) (NLC) (LVN)
β’ Proficient in Microsoft Office Suite
β’ Strong written and verbal communication abilities
β’ Minimum of 3 years of experience in direct patient-facing roles
