Licensed Practical Nurse

This is a fully remote position, open to applicants in Massachusetts.

📋 Description

• Deliver professional nursing care to patients in their residences under the supervision of a Registered Nurse (RN).

• Develop and execute personalized care plans in collaboration with patients, their families, and the healthcare team.

• Provide education to patients and their families on managing diseases, exploring treatment options, and implementing self-care strategies.

• Keep precise patient-care records that log visits, interventions, and outcomes.

• Maintain timely communication with patients, physicians, clinical managers, and other healthcare team members regarding needs and changes in status.

• Engage in care coordination and discharge planning activities.

• Facilitate patients' recovery, rehabilitation, and autonomy within their home environment.

⛳️ Requirements

• Valid nursing license in the state of practice.

• Active driver's license.

• Current auto insurance.

• Dependable transportation.

• Up-to-date CPR certification.

• Minimum of two years of experience as an LPN/LVN in a clinical environment.

• Previous nursing experience in a Home Health or Hospice setting is preferred.

• Participation in Humana's Tuberculosis (TB) screening program is mandatory.

• Must uphold personal vehicle liability insurance in accordance with state minimum limits or specified higher limits.
