Licensed Physical Therapist – Care Navigator

This is a fully remote position, open to applicants in California.

📋 Description

• Deliver remote monitoring services as patients advance through home-exercise therapy programs.

• Guide and assist users in therapeutic exercises, neuromuscular re-education, and functional training via the technology platform.

• Offer continuous feedback to evaluating clinicians regarding patient progress outside of the clinic.

• Keep essential records and notes during the remote monitoring process.

• Contribute to enhancing platform processes and functionality.

• Efficiently organize and manage a caseload.

• Recognize patient needs and challenges, implementing interventions that promote behavior change.

• Build trusting relationships with clients and patients.

⛳️ Requirements

• Completed an accredited physical therapy program.

• Active Physical Therapist license in California.

• A minimum of 2 years of experience in musculoskeletal health is advantageous.

• Proficient with technology and comfortable navigating EHR, messaging, and scheduling systems.

• Experience in behavioral modification is a bonus.

• Health Coach Certification is a plus.

• Exceptional communication, interpersonal, and presentation skills.

• Ability to gather and present significant data to inform decision-making.

• Strong skills in relationship-building and active listening.

• Proven patient-focused approach and enthusiasm for user experience.

• Capable of multitasking and managing various types of users.

• Ability to work independently with minimal supervision.

• Flexibility to collaborate across functions in a dynamic environment.

• Independent and proactive work style with quick and adaptable thinking.
