This is a fully remote position, open to applicants in California.
• Provide well-coordinated, patient-focused virtual Care Management via telephone or video that enhances members' health outcomes.
• Collaborate with members and our varied care team to develop significant care plans that help members reach their goals.
• Assist members in navigating intricate medical conditions, treatment options, benefits, and the overall healthcare system.
• Work alongside the members' local providers to ensure seamless care coordination.
• Deliver compassionate, ongoing follow-up care, fostering supportive relationships.
• Offer assistance during acute healthcare incidents, such as hospital admissions and rehabilitation stays, delivering coordinated Case Management to aid the member and their family.
• Organize essential resources that comprehensively address members' issues, whether they are clinical or social.
• **Bachelor of Science in Nursing (BSN).**
• A minimum of 5 years of nursing experience.
• At least 2 years of experience in care management.
• **Must reside and be based in California.**
• **Active California Nursing License.**
• Willingness to obtain (and maintain) licensure in multiple states.
• Work hours from 9 AM to 6 PM PST.
• Comfort in discussing various medical conditions and experience with diverse age groups.
• Spanish speaking is a plus.
• Experience in remote work and strong proficiency in using various computer and medical record systems.
• Display empathy, as our work involves assisting patients and their families during difficult times. You need to reassure patients that help is available.

• Adhere strictly to security and HIPAA regulations to safeguard our patients' medical information.
• Maintain a pleasant demeanor, be responsive, and show willingness to collaborate and learn from the team.
• A significant amount of time will be spent communicating with patients and families, as well as with colleagues. Thus, the ability to gather clinical histories, respond to questions at a patient level, and summarize findings effectively is essential.
• Proficient in documenting medical information in clear, patient-centered language.
