Licensed Clinical Social Worker – EST

📋 Description

• Work collaboratively with a diverse clinical team to provide comprehensive, patient-focused palliative care.

• Perform in-depth assessments, including the evaluation of psychosocial distress, mental health requirements, caregiver strain, social determinants of health, and risk factors affecting patients with serious illnesses.

• Analyze assessment and screening results to determine priority needs, emerging risks, and suitable interventions within the palliative care framework.

• Engage in serious illness discussions with patients and their families, facilitating advance care planning conversations and appropriate documentation.

• Offer clinical recommendations and escalate issues to physicians, advanced practice providers, and other clinicians following established escalation protocols.

• Create, document, and contribute to clinically relevant interventions within personalized care plans, ensuring they align with patient goals, values, and value-based care outcomes.

• Actively participate and provide clinical insights during interdisciplinary rounds and case conferences, promoting coordinated, goal-aligned care for an optimal patient and family experience.

• Provide short-term clinical counseling, psychoeducation, and emotional support to patients and caregivers dealing with serious illness, disease progression, and end-of-life matters.

• Serve as a clinical resource for the care team by identifying psychosocial barriers to care, suggesting evidence-based interventions, and facilitating transitions across care settings.

• Utilize Tuesday Health’s electronic systems, tools, and technologies to document clinical assessments, interventions, and care plans.

• Build and maintain therapeutic, trust-based relationships with patients and their families.

• Collaborate with the multidisciplinary team to enhance quality of life, reduce avoidable healthcare utilization, and support holistic outcomes, including timely and appropriate hospice transitions for patients with serious illnesses.

• Support the interdisciplinary team through debriefings following emotionally challenging cases and by facilitating periodic memorial rounds for the team.

⛳️ Requirements

• Master’s in Social Work (MSW) from an accredited institution.

• Active, unrestricted Licensed Clinical Social Worker (LCSW) license in the relevant state(s).

• Significant clinical experience in palliative care is essential, including expertise in advance care planning and serious illness conversations.

• Experience in an interdisciplinary care model, collaborating with physicians, advanced practice providers, nurses, care managers, health plan partners, and community-based resources.

• Proven ability to exercise independent clinical judgment, including knowing when and how to appropriately escalate concerns or respond to urgent or emergent situations.

• Strong verbal and written communication skills, with the capability to clearly document clinical assessments, interventions, and care plans.

• Excellent organizational and time-management skills, with the ability to juggle multiple priorities while ensuring a high-quality patient and family experience.

• Proficiency in utilizing electronic health records (EHRs), clinical documentation systems, and digital tools, with the agility to adapt quickly to new technologies as the organization evolves.

• Capacity to perform effectively in a fast-paced, changing environment, demonstrating flexibility, resilience, and comfort with ambiguity.
