This is a fully remote position, open to applicants in United States.
• Take ownership of the entire clinical data pathway, encompassing the entry of studies into hospital systems and the delivery of results back.
• Develop and sustain on-premise DICOM ingestion utilizing a pynetdicom C-STORE service.
• Compile studies, manage senders that do not indicate completion effectively, apply backpressure, maintain compressed transfer syntaxes, and spool data to disk during link interruptions.
• Create cloud DICOMweb STOW-RS ingestion featuring streaming and bounded-memory multipart parsing.
• Implement per-part study-UID validation and guarantee one canonicalization along with one inference firing per request.
• Ascertain customer tenancy without any silent defaults.
• Enforce DICOM PS3.15 confidentiality regulations and ensure a fail-closed approach before data exits the customer’s network.
• Maintain a secure re-identification store protected by a customer-held key.
• Develop outbound JSON callbacks, dictation-system autotext, and HL7 v2 ORU^R01 over MLLP.
• Tailor integrations to match each customer’s interface engine requirements.
• Offer per-study tracing and monitor the health of three-component integrations.
• Verify drift between expected configurations and deployed versions.
• Bring both gateway repositories under continuous integration within the first week.
• Engage in hospital IT and clinical security assessments.
• Provide detailed documentation and responses regarding transport, retention, incident management, and system behavior.
• Troubleshoot integration failures using packet captures, memory profiles, and protocol documentation.
• Be available during the customer’s clinical hours and travel as needed for on-premise installations.
• Proven experience in implementing protocols directly from their specifications.
• Capability to operate software within infrastructure without direct SSH access.
• Skill in debugging bare 502 errors through packet captures or memory profiles.
• Proficiency in reading and applying protocol specifications accurately.
• Adherence to a fail-closed approach when managing patient data.
• Capacity to produce precise and reliable documentation for hospital IT reviewers.
• Experience with Python, FastAPI, and pytest.
• Familiarity with pynetdicom and pydicom.
• Knowledge of DICOM PS3.15 and PS3.18 standards.
• Understanding of DICOMweb STOW-RS.
• Acquaintance with HL7 v2 over MLLP.
• Experience with AWS services such as S3, SQS, Cognito, ECS/Fargate, WAF, ALB, CloudWatch, and Secrets Manager.
• Proficiency with Terraform and Docker.
• Ability to write shell scripts that function on secured hospital systems.
• Willingness to participate in hospital IT and clinical security evaluations.
• Availability during the customer’s clinical hours.
• Willingness to travel occasionally for on-premise installations.
• Helpful but not mandatory: Experience with DICOM, PACS, VNA, teleradiology, HL7 interface engines, medical imaging open-source projects, networking, embedded systems, payments, healthcare or regulated environments, de-identification, privacy engineering, confidential computing, or hardware attestation.
• Approximately 10% discretionary bonus for compensation based in the US.
• Equity may be extended to top candidates in the US.
• International offers will be cash-only and will not include equity.
• Remote work is available from the candidate’s home country for international hires.
