Intake Coordinator, Licensed Practical Nurse (LPN)

This is a fully remote position, open to applicants in Iowa.

📋 Description

• Receiving transfers from the Patient Enrollment team to carry out initial health assessments for newly enrolled patients within our network.

• Addressing patients' inquiries and fostering an open dialogue to comprehend their needs.

• Identifying and coordinating community resources that would enhance patient care.

• Delivering patient education and promoting health literacy regarding the management of chronic conditions.

• Assisting with medication management, including recognizing potential medication issues, ensuring adherence, and coordinating refills.

• Aiding in the timely delivery of services for your patients, including Home Health, DME, Home Infusion, and other essential needs.

• The individual in this role must be able to swiftly adjust to a dynamic work environment, as this position involves being on the phone for the majority of your shift.

• Active Compact/Multi-State license (LPN) (LVN)

• Technical proficiency in Microsoft Office Suite

• Outstanding written and verbal communication abilities
