This is a fully remote position, open to applicants in New Mexico.
• Receive referrals through phone calls or in-person visits for the specified department.
• Communicate benefits and services to patients, their families, and community contacts.
• Help identify alternative services for patients who are not eligible for hospice care.
• Coordinate the scheduling of initial nursing assessments and admissions.
• Maintain admission packets and ensure the availability of necessary forms for the admissions staff.
• Assist community outreach representatives with marketing initiatives and managing referral source relationships.
• Provide telephone and clerical support to team members as required.
• Keep accurate clinical records and monitor patient goals and progress.
• At least one (1) year of experience in home care, Home Health, or a related healthcare field is preferred.
• Ability to complete documentation accurately, legibly, and promptly.
• Capability to professionally manage stressful and sensitive situations.
• Strong interpersonal, verbal, and written communication skills.
• A valid driver's license and proof of insurance are required.
