• Assist patients in transitioning from hospital to home by coordinating specialty and home infusion services.
• Work in collaboration with hospitals, clinics, physicians, and home health agencies to guarantee timely and effective home-based care.
• Oversee the complete referral lifecycle, which includes insurance verification, scheduling of home health services, and coordination with operational teams.
• Provide education to patients and caregivers about therapy expectations and financial responsibilities with empathy and clarity.
• Develop and maintain robust relationships with referral sources while promoting growth across designated hospitals and clinics.
• Recognize patient and family concerns and facilitate issue resolution to enhance satisfaction and ensure continuity of care.
• Keep precise documentation and uphold patient confidentiality at all times.
• Engage in necessary meetings, in-services, and training sessions.
• Adhere to established administrative, HR, and organizational policies.
• Strong focus on customer service with the capacity to identify and address the needs of patients and referral sources.
• Exceptional communication and interpersonal skills.
• Strong organizational, analytical, and problem-solving capabilities.
• Ability to juggle multiple priorities and work autonomously in a dynamic environment.
• Proficiency in Microsoft Word and Excel.
• Bachelor’s degree or equivalent experience is preferred.
• Experience in managed care, insurance, or healthcare coordination is preferred.
• Experience in project management or public speaking is preferred.
• Ability to work collaboratively with diverse stakeholders across various healthcare settings is preferred.
