In-Home Nurse Practitioner, Physician Assistant

This is a fully remote position, open to applicants in Virginia.

📋 Description

• Conduct in-home health assessments for members in their residences.

• Engage in approximately 45-minute sessions involving personalized health discussions.

• Evaluate medications and review medical history.

• Carry out physical assessments.

• Administer additional diagnostic tests as prescribed by the health plan, which may include diabetic eye exams, spirometry, and bone density screening.

• Utilize an iPad and follow clinical workflows for each assessment.

• Establish trusted relationships to enhance health outcomes.

• Travel across the nation to connect individuals with suitable care.

• Offer Signify Health services, encompassing Diagnostic & Preventive Services products.

• Undertake local travel and within licensed states as necessary.

• Facilitate virtual visits when required.

• Primarily operate in the private homes of health plan members.

⛳️ Requirements

• Possession of an active, unrestricted license(s) in the relevant coverage area(s).

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Capability to travel locally and within licensed states as required.

• Ability to conduct virtual visits as necessary.

• Competence to safely perform duties in homes that may have pets, secondhand smoke, and other potential airborne allergens or irritants.

• Qualification as a Nurse Practitioner or Physician Assistant.
