In Home NP/PA – Part Time

This is a fully remote position, open to applicants in New York.

📋 Description

• Deliver Part-Time In-Home Health Assessments within the residences of our members.

• Conduct visits that last approximately 45 minutes, which include a review of medication, medical history, and a physical examination.

• Carry out additional diagnostic tests as requested by the health plan, such as diabetic eye exams, spirometry, and bone density screening.

• Utilize an iPad and follow clinical workflows during each evaluation.

• Address health inquiries and offer health education to members.

• Travel nationwide to connect individuals with suitable care options.

• Conduct virtual visits when necessary.

• Provide services under Signify Health, including Diagnostic & Preventive Services products.

• Travel locally and within licensed states as required.

⛳️ Requirements

• Possess an active, unrestricted license(s) in the relevant coverage areas.

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Ability to perform duties comfortably and safely in homes that may have household pets, secondhand smoke, and other potential airborne allergens or irritants.

• Willingness to travel locally and within licensed states as needed.

• Qualifications as a Nurse Practitioner or Physician Assistant are essential.
