In Home NP/PA – Part Time

This is a fully remote position, open to applicants in New York.

📋 Description

• Conduct health assessments in the homes of members.

• Foster engagement with members and establish trustworthy relationships.

• Examine medications and review medical histories.

• Carry out physical assessments.

• Offer supplementary diagnostic and preventive services, such as diabetic eye exams, spirometry tests, and bone density screenings.

• Execute virtual consultations as required.

• Travel locally and within licensed states as necessary.

• Assist in identifying connections to the appropriate care and addressing gaps in care.

• Utilize an iPad and a user-friendly clinical workflow for each assessment.

• Refrain from prescribing medications, ordering lab tests, or modifying existing treatment plans.

⛳️ Requirements

• Active and unrestricted license(s) in relevant coverage areas.

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Qualification as a Nurse Practitioner or Physician Assistant is required.

• Capability to safely and comfortably perform duties in homes that may have pets, secondhand smoke, and airborne allergens or irritants.

• Willingness to travel locally and within licensed states as necessary.

• Ability to conduct virtual visits when needed.

• Proficiency in using an iPad and following clinical workflows.

• Competence in performing medication and medical history reviews, physical evaluations, and necessary diagnostic tests.
