In Home NP/PA

This is a fully remote position, open to applicants in New York.

📋 Description

• Conduct in-home health assessments for members at their residences.

• Perform visits lasting approximately 45 minutes.

• Interact directly with members to address their health inquiries.

• Review current medications and medical histories.

• Execute physical assessments.

• Carry out additional diagnostic tests as prescribed by the health plan, such as diabetic eye examinations and spirometry.

• Utilize an iPad and clinical workflows for each assessment.

• Establish trustworthy relationships and enhance health outcomes.

• Travel across the country to connect individuals with suitable care.

• Conduct virtual consultations as necessary.

• Offer Diagnostic & Preventive Services, including spirometry, diabetic eye examinations, and bone density screenings.

⛳️ Requirements

• Active, unrestricted licenses in relevant coverage areas.

• Board certification is mandatory.

• Multi-state licenses are preferred.

• Willingness to travel locally and within licensed states as required.

• Capability to conduct virtual consultations as needed.

• Ability to safely perform tasks in private residences that may feature household pets, secondhand smoke, and other airborne allergens or irritants.

• Qualification as a Nurse Practitioner or Physician Assistant.
